Improving the experience of various forms of transitions for residents and their families.
In undertaking the quality improvement initiative component of the programme, Cohort 8 explored the notion of contributing to a set of guidelines related to a person transitioning from living at home to a care home setting.
This includes, a person transitioning from their home/hospital to a care home, or a resident coming back to the care home after admission to hospital or the Emergency Department (ED).
We engaged the expertise of Dr Marie O’Neill, Lecturer in Nursing Ulster University whose research focus is on older adults’ experience of transitioning from living at home to a care home. Marie met with the participants to share her research findings and support them with their initiative.
The light touch approach to this QI initiative was that Cohort 8 decided to undertake discovery work with their own residents and relatives about transitions.
This initial work forms the baseline on improving the experience of transitions for residents and their families in Cohort 8’s care homes.
